You have the right to:

- Request corrections to your health information
- Receive confidential communications
- Request restrictions on certain uses or disclosures
- Obtain a list of disclosures we’ve made
- Receive a copy of this notice
- Designate someone to act on your behalf
- File a complaint without fear of retaliation

2. Our Responsibilities
Advanced OHS is committed to:
- Maintaining the privacy and security of your protected health information (PHI)
- Providing you with this notice of our legal duties and privacy practices
- Notifying you promptly if a breach occurs that may compromise your information
- Following the terms of this notice and updating it as needed

3. How We May Use and Share Your Information
We may use and disclose your PHI:
- For treatment: To coordinate care with our medical team and other providers involved in your recovery
- For payment: To bill your insurance or employer/third party administrator for services rendered
- For healthcare operations: To improve our services, train staff, and manage clinic operations

We may also share information:
- With public health authorities for workplace safety and disease control
- With law enforcement or legal entities when required by law
- To prevent serious threats to health or safetyFor workers’ compensation claims
- With your employer or third party administrator (TPA) when authorized and relevant to occupational health assessments and/or treatment
- With family or designated representatives involved in your care (unless you object)

We will not:
- Use or disclose your information for marketing purposes without your written authorization
- Sell your health information under any circumstances

4. Telemedicine and Digital Communications
Advanced OHS offers telemedicine services. We use secure platforms to protect your PHI during virtual visits. SMS or email communications will not include sensitive health details unless you have provided explicit consent.

6. Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
